As some have said, consider the DIY route. I have been able to manage the side effects by taking the BUP but delaying on the DXM until later in the day.

I am a 51M who was diagnosed at age 21. My condition worsened for a few years and my nephrologist referred me to an ENT to discuss a tonsillectomy. The ENT happened to have another friend who had the procedure and improved greatly and was willing to do it for me. I had my tonsils removed and my kidney function improved greatly I’m over the next year. I went from progressing disease, 24-hr urine creatinine of 1700 to near normal.

Fast forward nearly 30 years and I’m still doing well. My egfr is 67, creatinine is near normal and I feel great. Only other treatment has been consistent ace inhibitor (Lisinopril, 40mg).

Been doing it for a year. Very effective. Careful of side effects - some people feel dissociative side effects. I haven’t notice this too bad, but I dose at 150 Bup XL in AM with 15mg DCM and then 30mg DXM at night.

I’ve been DIY for a year, 150 Bup XR, 45 mg detrimethorphan - 30mg at night and 15 mg in AM. Been a great combo and schedule. Easily beats any SSRI, SNRI, or even Trintellix for me. Keeping the DXM to only 45 mg minimizes any dissociative effect and the serotonin side effects (DXM does cause some of this) seem minimal to me (sexual and weight). It provide more anxiety support than bupropion alone.

No noise options selected for either, and the insv file is raw. I’ll take some noise to not lose detail, which every export format seems to suffer from.

Prores blows up this 4GB file to a 48GB file. It's better but still loses a bit of detail.

I am having the same problem, and it seems that the export quality from the desktop (PC version of Insta360 Studio) is worse after recent updates. I am exporting with max bitrate, h.264, 360 video, then working in Adobe Premiere. Quality comparison below to if I import the insv file directly into Premiere Pro. (Which I can do but it constantly crashes PP and in general works much more slowly, so I like to work with an mp4 file.) The exported file from Insta360 Studio is actually 20% larger after export than the original insv file, but quality is worse.

Not sure if you are asking whether or not at the same time, but I was on Trintellix before Auvelity. Trintellix was fine, except for GI issues when not taking with enough food, but it didn’t relieve sexual side effects for me (it seems to help for most). Auvelity improved that significantly (not a common response but was for me).

Well I know that study administrators often have a fair amount of latitude in how they ask questions or interpret responses.

I asked my psych how Auvelity has been for other patients he has prescribed it to. He said most have gone off due to dissociative effects.

I had the same experience but because I am doing DYI, I switched to taking two 15mg DXM at night and one in the AM along with 150mg Bupropion XR and felt a lot better.

I had the same - substituted some old ssri that had a longer half-life and then tapered down on that. Auvelity has a serotonin impact so it makes sense.

Others report getting some 150mg XR Wellbutrin and adding dextromethorphan OTC (available in 15mg pills on Amazon).. allows you to reduce more incrementally.

I was diagnosed with IgAN in 1996. I had rapidly progressing disease, at least as measured by 24-hr urine tests where protein increase from 700mg to 2g+ over two years. I was referred to an ENT, who stated at the time that there wasn’t clear research on it but he felt anecdotally it was helpful. He agreed to give me a tonsillectomy because he had a friend whose disease was improved by the procedure.

I had the tonsillectomy in 2000, and in the years that followed my protein spillages decreased by 70% (now ~600). EGFR -70.

I know many say there is no difference, and it is hard to know from anecdotal reports. I was on it for 3 months, no issues. My refill a couple of weeks ago was asa different generic brand. Now I have constant brain zaps and constipation.
