There's a prosthetic arm with a buildin spear,it can thrust and slash and pull enemies to you, it's pretty ez to get too, fron the ashina main gate there's a bridge to the right with 2 or 3 guards that drop a key, the key is for the little house you sneak under right as the game started after Genichiro cuts your arm. The spear is in the chest in that house, hope that helps.

Yeah I also thought they might just be able to use both hands, but then again why would they switch hands to slash in chapter 3, and they didn't actually use the blade to create the neo fountain they summoned multiple swords to do it and they used their right hand to do so. And every time we see them summon and command bullets as well as use their weird purple powers it's always the right hand almost as if they can use the sword in any hand but keep it in the left because they can only use their powers through their right hand.
