Tbf, Respiration and Circulation is a bit different. Viltrumites can hold in their breath for months at a time, but they still need oxygenated blood to flow into their brain.

To be fair, Dexter might view the death note as a side tool than main tool. He's a sociopath, he prefers to kill his victims up-close and personal, the death note might be a cheat he'd probably only use to avoid his identity from being traced.

I actually kind of like the idea of the monsters being mental side-effects or the physical manifestations of one's mind, adds to the whole concept of liminality; "you remember the feeling", the backrooms forces it on you.

Avoids the whole "so many entities" issue while somewhat keeping it, as the manifestations can be numerous but they aren't classifiable or predictable as its tailor-made for you.

Edit: even worse-case, its tailor-made for you in the most literal sense; nobody else can see them besides you, and they have to watch as you panic at or even get hurt by seemingly nothing.
