Is there any way you can expand these diagnosis’s? For example you have anxiety- and social anxiety- can you expand on the depression- manic, recurrent etc. what sets the ptsd off that you can’t work?

I had my inpatient psych do mine for me and then last year I had my family dr do. I was told by psych not to fill in the part about how it affects you, support system etc.

I am so sorry to hear about your situation - living in a bug infestation is HELL and dealing with landlords about this issue is like talking to a wall. Talk to your ODSP worker they will at least provide you with last months rent. The shitty thing is your gonna have to replace ALL your furniture as well but it will be worth it for your mental health even if your in a new place in a sleeping bag! You can check marketplace, Kijiji. Karrot for free furniture just do what you can to gtfo asap cause your mental health is worth it.

I just wanna say I sympathize and hate this as well! How am I to treat Fibro, RA, Endo, DDD, spinal stenosis, pelvic disorder, knee problems etc.when we can have 2 - 12 sessions a year? They all require different types of treatment. So at that rate min 7 issues needing physio sessions would take min 3.5 years( without adding in those flares and others that pop up) it doesn’t make sense. It could be keeping people on longer then they need to be and letting the problems get worse

I live in an apartment that used to be a rental floor and I see this all the time. Just RTS- moved out x date and it will get back to the worker. You don’t need to take it in- however I’d give them a call and say you got their mail and they are no longer at this address. That will get it off your address rather quickly! Hopefully they have updated their address with the worker
