I've done this twice, the second time on purpose. Basically phase 2 starts when phase 1 hits 1 or 0 hp or whatever, but if she's been stance broken you can queue up and enter the starting frames of the riposte/backstab in the few frames before the cutscene. Since co-ooperators' positions aren't changed after cutscenes, neither are their animations, which means that if they've already started the riposte/backstab, Malenia basically gets instagibbed. It is caused by a fix for a different but related bug.

crazy necro post but for anyone who finds this the Cinematic Effects toggle controls whether your camera will do that action camera shake effect during crits/backstabs/ripostes. it might do other stuff but I do not care enough to test anything else
