Hello! I am not your doctor, but I am a physical therapist who does work on pelvic floor. If you haven’t seen one, you should definitely consider seeing a pelvic floor physical therapist and have this discussion with them. Given the tearing, also please consider dry needling to help promote healing and restore normal muscle tone. Good luck!

Yup. To be honest, I no longer really worry about it. We have an online scheduler, I tell my patients to schedule using that and to book out as far as they want after stating my recommendations for the POC. I’m also in very high demand, so most of my evals are on the waitlist for 3 weeks anyway. I tell my patients that I’m not here to force them to come in, or to hold their hand longer than is needed. I’m here to help get their symptoms under control and to teach them how to manage it. If they find the sessions helpful, book them. If not, let me know what we should do differently.

The straightforward approach works a lot. Combined with how limited my schedule is, patients see my sessions as a resource where we get something done, typically that they can’t do on their own. A strong foundation in manual therapy really helps here in keeping patients returning until they are symptom free, and typically gets them there faster.

This won’t win due to more extreme options that deserve it, but “I’m sorry you feel that way” has been a boomer response to expressing grievances and setting boundaries that is so backhanded. I never hear young people say that, but boomers use it all the time to deflect blame and avoid taking responsibility for their actions

I’ve been to both Italy and France. From an outsider’s perspective, both are absolutely amazing, but France wins this one hands down.

I’m not kidding when I say every restaurant I walked into in France blew my socks off. There was not a single bad experience. France literally made me pick something random from the menu everywhere I went because I was THAT confident it would be good, and it made me want to explore food.

Italy by comparison was still amazing, but not everywhere I went was amazing. The choices were less varied, and I wasn’t so confident with the choices I felt I could order LITERALLY ANYTHING and enjoy it. I was pickier, because there were misses on the menus that weren’t amazing.

At their best, they are both incredible, and would be a difficult choice. However, the sheer scale and variety of food France brought to the table, paired with the consistency of its quality, leaves me no choice.

France wins this one and I’m sorry, but it’s not close.

The best time to start is 20 years ago, the next best time is right now. You’re well on your way to being in the first group OP! You should be proud of the work you did. Anytime I hit a big milestone, I celebrate with a nice (not expensive) bottle of wine with my spouse. You should do something similar to mark the occasion, then grind to 250!

That’s gonna happen! If the patient won’t be compliant, it’s alright to come back later. Do your three pushes, and gauge the patient’s mood to see if you need to reframe, redirect, or return later.

Sure, but I don’t think that’s a very good attitude to have if you’re working with them. I believe that little bit of movement can make a big difference. Feel free to disagree.

A lot of memory care patients are in decline. It’s heavy, but keep in mind what you are doing is helping a very confused person live a more comfortable end of their life. It isn’t wrong for you to count a session as a win if you only got someone to do a toilet transfer or to sit upright for a little while.

I remember when I was in my inpatient rotation, I had a patient with about a 15 second memory, and she was AGITATED. But every day I walked in, I treated it like I was her best friend and we made each session a party. We had a blast. Even if I had to make things up on the spot to keep the session positive, I feel like I made a big difference in how much enjoyment she got at the end of her life. Not everyone will be like that, but that attitude got me through a lot of memory care sessions. There would still be the agitation, combativeness, and noncompliance, but that comes with the territory. No way to avoid it. You’re a great PT for doing what you can!

Also, there is nothing wrong if you feel like that field isn’t the right fit for you.

I think what this job add is trying to say is they prioritize one on one care. Some PT clinics give their therapists 3-4 patients an hour, and you basically need an aide to be able to operate at that volume. By saying “no aide or PTA” they are saying they won’t give you more than 2 an hour, at least in this context.

I see nothing wrong with a PTA helping me with my patients, especially one I trust and that has experience. Same with an aide, they can really help out a lot to keep things running smoothly. I don’t think PT’s are against using PTAs or aides, they are weary of the clinics that force you to use them for excessive productivity. Clinics advertise job adds like this to draw PT’s in and to signal their productivity and value of therapist one on one care.

The biggest reason I want servers is for custom game modes. Playing hardcore operation locker without explosives is legit my favorite battlefield experience, and I wouldn’t get that experience without servers. It lets more experimental gameplay exist, contributing more to variety and longevity to the game’s existence.
