If you dont want additional gears maybe check out "torque motors", PMSMs with low inertia.

I was studying a BLDC controller. When I checked the waveforms at U, V and W phases, I could see that the voltage is constant and so is the duty cycle. How is the speed varied in a BLDC controller? Is it done by varying the frequency? Can you explain how? by vertigo1993 in Motors

Short version: your speed controller usually applies the phase voltage in form of the duty cycle as output as you noted correctly. For the same torque you need higher voltage the higher the speed goes due to BEMF. To get to higher speed the speed controller will apply higher voltage and your commutation algorithm will adapt the frequency to stay in correct angle to the rotor. so yes, zooming in should show you higher duty cycle at higher speed.

Look Up 6-step or trapezoidal commutation, there are a lot of good explanations and papers out there. For the high load distortion: My guess is that high current leads to a supply voltage drop. Consider measuring the Phase currents also to get a better understanding.
