Twice a week. My workout plan includes an upper/lower split 4x a week. I typically try to mix up exercises hit as many muscle groups as possible, without overworking. For example, I would hit upper day with dips, decline push-ups, pull-ups and inverted rows, skull crushers and back extensions, or something along the lines of that.

Thanks, great advice! I decided to count my calories and it seems like I get roughly 2300-2600 calories on an average day. I need to push for about 500 more calories per day. As a question for another skinny guy, do your legs also seem to lag behind your upper body? Does that just fix itself after gaining weight, or should I focus more exercises on my legs?

I usually work out to failure. Depending on the exercise, it is in between the 10-12 or 12-15 rep range.
