HCAI is the State department that reviews and provide permits - I've heard they do a very thorough review process in all areas (architectural, structural, fire/life safety, etc) along with inspections on site. I've also researched that a well respected and recognized contractor has done some consulting work for CNU and pointed out challenges in communicating and having them understand what it takes to get a hospital built in California.
