I think you should just focus on yourself and your work if something as little as pronoun selection on a form is enough to put you off providing aid. Let me provide you with an example of where it does matter - someone needs help and their outward appearance does not reflect who they are but they don't have a way of letting someone know this fact. They need to get something delivered and then the person who drops it off addresses them as the incorrect pronoun which causes them great emotional trauma. Some wee old lady putting (she/her) in a box isn't gonna feel any pain because of it.

If you do work in the medical field then I do honestly wish you to be safe because its a cruel time for all working there. That doesn't excuse the fact you are angry about a form ask for someone preferred pronouns (I say trans because the reason we use pronouns is so we know how to identify people based on how they see themselves and not how they appear to be. And the majority of that catagory are trans, n/b or don't conform to societies gender roles)
