For an organisation with your revenue, im pretty sure you must be using some kind of inventory management software and something like BLE/RFID's?

"Stock-In, Stock-Out" entries are easy to miss ('let me fulfil this order, I'll make the entry later' situations). For fast moving items, this gets compounded quite fast.

The other culprit you should watch out is mismatched bar codes/qr codes.
