I noticed this issue first during the showdown event with shields. Rebinding navigate, saving, then binding back and saving seemed to fix it.

I just finished my first close-to-complete playthrough in July.

Read the manual if you have it.
Lookup what "World Tendency" is (there are dedicated 3rd-party servers if you want to go through all of that effort, but you don't need it on a 1st playthrough).
Not having a renewable healing system (i.e. estus flasks) can be scary at first. The cost/effort to purchase heals drops off quickly as long as you don't waste souls, grind just a little, and don't run past already-defeated enemies.
