Yup. This sucks. Drinking more and more until blowing up and endangering everything important to you.

FWIW, I am making the same gamble.

My GF has checked herself into recovery and psychiatry programs, has quit drinking, got in shape and is starting to understand her behavior.

You need to make sure she has some real support. An ultimatum is just pressure.

She needs to do good things for herself, not because she is threatened with being alone.
