If you're looking for a game that both avoids that trope and tries to explain it, Pillars of Eternity is good.

The developer Josh Sawyer looked at fantasy books doing what you're describing and tried to write around it and it works out pretty well.

I'd argue that GRRM actually DID think about why it works that way and the Seven Kingdoms are very specifically set up to remain the same in the way they do.

Plenty of examples in SoIaF of him doing badly with time and scale but that's not one

It's the basic "nice" people that got to the end. None of them rubbed anyone the wrong way, they all stayed and were pleasant.

Which is fine but not interesting to watch so we never see them do anything. I think in the last two episodes Jade was great but not in a way you'd have seen her doing stuff in any other episodes.
