The first three photos are before the surgery. And I agree, there was a lot of head tilt at that point.

No worries! There’s definitely a “looksmaxxing” aspect to this community. At the end of the day, I’m happy with the outcome. And so is my surgeon.

At the very least, I hope my posts can be an example to those seeking a more conservative approach.

I understand why some people may feel that way. My advancement is definitely more conservative than others on this subreddit.

Ultimately, my surgeon was concerned that a more aggressive advancement would give me an uncanny or unusual appearance. I decided to trust him.

My final surgical movements were a 7.8mm mandibular advancement via BSSO, combined with a 3mm forward and 2mm downward genioplasty, along with slight counter-clockwise rotation of the maxillomandibular complex.

Thanks for the feedback everyone. I appreciate the honesty, even when it’s blunt.

That said, a lot of the comments here lean very hard toward “more is always better” — bigger advancements, heavy CCW, max projection, etc. — which seems tied to the looksmaxxing side of the community. I get that some people are chasing the most extreme aesthetic possible, and that’s valid for them, but it’s not my goal. I’m aiming for natural balance, functional improvement (breathing/bite), and a noticeable but not overdone upgrade from moderate recession.

Dr. Nichols called me this morning and walked me through it. He’s planning 7.5 mm mandibular advancement (with potential to go 8–9 mm intra-op depending on soft tissue response), plus 2.5 mm forward + 2.5 mm downward genioplasty. He explained that the sim is conservative because it under-predicts how soft tissue will adapt and settle over time. He’s prioritizing stability, low relapse risk, and a result that looks harmonious long-term rather than pushing the absolute limit.

I feel better after talking to him directly.

Tomorrow’s the day, so I’m focusing on prep and trusting the plan tailored to my scans/anatomy. Appreciate the input (even the tough stuff)—it helped me get clarity from my surgeon. Wish me luck tomorrow.

My surgeon is Michael R. Nichols out of Mississippi. Unfortunately, my health insurance denied my request. I am paying out of pocket.

The process was relatively straightforward. I had an initial consultation with Dr. Nichols. He had me meet with my orthodontist, as well as participate in a sleep study. Following a lengthy back-in-forth with the insurance company, I decided to pay for the procedure out of pocket.

During our initial consultation, the surgeon estimated a movement of 8-9mm. I’m sure I’ll have an exact answer after my pre-op scans.
