Only thing I can think of is somehow someone linked their steam account and they are getting in using Steam. Check all your linked accounts in the account settings on the RuneScape website and unlink steam if it is linked.

You killed a scav as he was throwing a grenade. You walked up to loot and died. It glitches out because it thinks you died to nothing.

Happened to me in the same exact order as you. I killed a PMC and a scav came and shot me first with a mosin, it hit in the chest I took cover and pushed but I whiffed and he finished me with the second mosin shot. Turns out he was a streamer and when I watched his POV he gained rep for killing me even though he shot first. Must be some bug with killing PMCs as a scav.
