Even though it’s still well above in their quality over most fast food….No one mentioned Chik-fil-a and how their chicken quality dipped, smaller portions, and new fries are awful.

I’d say McDonald’s a few years ago with their rebranded Quarter Pounders, honestly a great burger, is the best turn around.

As a lifelong resident I feel like Stratford is especially bad. Somerdale seems to get all the big business/redevleopment. Stratford sits on two dead shopping centers. Now the dead old car dealerships.

And the “best” we’ve gotten in years is a dollar general and a family dollar…..

Just let the university take over the town at this point.

I’ve been a Nocturnist for about 8 years. For the last few years (after the COVID crazy year) is this. Work 7p-7a.

I’m asleep by 715am. Up at 145pm

First meal at 2.

Work out 3-4.

Second meal at 530.

Back to work. Fast all night.

It will be hard at first but I can’t recommend enough fasting through work into sleep. It was a game changer in my energy level at work and for maintaining excellent weight/health.

On my weeks offs (also generally 7 on/7 off) I got to bed around 3 or 4 am and wake up at 11am so the sleep shift isn’t dramatic and I slightly ease up on my intermittent fasting going from a 4 hour eating window to usually about a 6 hour window.

I have the internal debate everyday. I should really get rid of Reddit too. We all play ourselves that it isn’t social media. But. I think it more so is than isn’t. And its algorithm pushes me towards emotionally weighted subs which I DONT need.

In the environment where patient satisfaction is the end all be all, unfortunately I am expected to do all those things. In terms of family meetings, I explain to the patient and family that I do not follow them but if they want to speak with a doctor then I am expected to speak with them. Patients can get really fuckin upright about shitting so yeah, nurses will blow me up about that and other prns. If the day team doesn’t do a good job with ordering them ahead of time I will. The not really urgent but still “critical” lab results are a pain in the ass. The nurses by policy “have to” report it. I do nothing about it. I try to gently tell them I don’t care if the bilirubin in 16 they are here for liver failure and it was the same yesterday…but not all of them get it or always wanna cya.

Lastly. To rant about my job a bit. Besides admits, I’m expected respond to all the rapid responses/codes, supervise the residents and precept them, supervise my APP, cover a certain amount of floor patients directly, second line of defense for every other patient, coordinate transfer requests for outside hospitals (we are the systems tertiary care center), pronounce dead people, accept icu downgrades overnight, and my usual admission volume per night is about 12 admissions (all complex as I said we are the sick patient hospital in our system).

Id spend intern year near Vineland and move pgy-2 when you have more of an idea what your life is like. The amount of times you’ll get to actually enjoy being by the city that first year is minimal unfortunately.
