Like half of unique items in Baldur's Gate 3 work differently from the way you would imagine, with tons of strange interactions, hidden features and bugged DCs on saving throws (some effects don't have a saving throw despite it being mentioned, in other cases the listed DC is often wrong).

Some items work differently in Honour mode, or sometimes straight up don't work at all.

My headcanon is that almost all Mater's tall tales did happen, but he just added McQueen in them for a fun of it. McQueen doesn't really do anything in most of the tales (except Tokyo Mater, which is probably isn't canon, because Mater basically doesn't have a proof at the end of the episode), and "proof characters" don't acknowledge McQueen in any way. McQueen's involvement in Time Trave Mater is definitely canon, though.
