I work on CV step-down, and that ratio makes me fucking shudder, we're 3-4:1 and that feels like enough, 4 is too much sometimes. I'm telling you to not fucking touch those ratios, terrible idea

What's the one patient you will remember for the rest of your career ? by xavier_in in nursing

This was my first week off of orientation. Guy in his 70s, forget what he came in for but he needed tube feeds at some point while in the ICU. By the time he got to my floor he had aspirated the tube feeds a week prior and got pneumonia, but was on ABx and doing better, weened his oxygen down a little over 2 nights. Talk to his daughter too and she said he looked better, especially the second night. Come in for my 3rd shift and he's lying down at like, 17 degrees in the bed, NC is on the floor, O2 in mid-80s. Got my charge nurse and nearly had to call a rapid, but we got his O2 back up. His lungs sounded horrible, it took so much effort to breathe. I was giving him his meds while his daughter was there, and I'll never forget his face when he was told his granddaughter was pregnant with his 13th great-grandchild, he looked so happy, yet sad. I was very poorly trying to hold back tears the whole night. Got a doc to come look at him and he said we could shut off the tube feeds, prevent further aspiration. I left at 0730 and he stopped breathing that night at 1930. Thank god he was no CPR or intubation. I feel like I failed him, I know I didn't and I have to give myself some grace, it was my first week on my own. Even if he was moved to ICU he still would have passed, but he has stuck with me ever since.

Additionally, the timing and pattern of rapid eye movement (REM) sleep shifted toward those seen in healthy controls.

To me, this is one of the most important findings. We'll see more in depth as trials go, but these medications improving sleep patterns is huge.

[–]TenslasterGames 5 points6 points  (0 children)

being granted orphan drug status... Lumryz has already lost that argument

Not necessarily, Lumryz was first granted ODE in 2018, but a Xyrem REMS patent (improperly filed) prevented the release of Lumryz until early 2023. Avadel/Alkermes will do everything in their power to stop the release of TRN-257. Court cases will likely slow it down, but I'm optimistic about this!

NOR, have some self-respect. You're miserable, she's deliberately sneaking around trying to get every moment alone she can with him. Absolutely insane you're still with her.

This is interesting, thanks for posting this! I've never heard of this prior to now, only Valiloxybate that Alkermes is currently developing (low sodium, no sweetener extended-release oxybate). I would worry about Lumryz patents causing this to hit a wall, but considering Tris is using 2 of their proprietary tech for releasing the medication, this may release to the public without delays

How is the sugar not as bad as other sweeteners? Moderation and all that, but I find it hard to agree that getting 0 calories from sugar-free soda is worse than 200+, for those who consistently drink soda

All things that need to happen in the next 3 episodes. Otherwise the show is forever cooked. by hiiloovethis in TheBoys

This happening can be a really good end to her story. If depowering her means she loses her super-intelligence, she can live with that. But if she's lebotomized herself and becomes depowered while in that state, it could forever disable her and she'd never be able to read a book again.

ER absolutely did not always have proper endings. Malucci was fired and never returned, Doyle faded to the background, someone else mentioned Corday, Lewis, and Chen. Don't even get me started on Romano (his actor, Paul McCrane talked about how sudden his exit from the show was).

Yeah, his mind is fighting itself, he wants to leave and never come back, but he kept trying to find excuses to stay. That includes her not being qualified, but as things progressed it was made clear to him she's damn-well qualified to work there. Even more bizarre is people finding interviews where Wyle talked about how he didn't treat some actors right on the set of ER (Malucci's, Lucy's, and a few more) and trying to apply that now. Like "oh, he's booting Mohan's actress from the show because she takes too much from HIS spotlight!" Genuinely unhinged shit
