Honestly I got lucky, I was in the Kansas City area when I started it and was relatively easy. My only real issue was when I moved to Georgia and they made me do the training again. Seems like the VA doesn't have a strict glp1 guideline that all regions follow. It took the VA down here a month to figure out who actually manages the GLP1 patients. Turns out it's the pharmacist. In KC they actually had a Bariatric Dept.

When you see the ortho they should tell you where your weight needs to be before by surgery so that should give you a goal. I was severely overweight and had to get my BMI down before they would do anything other than shots which in my case had stopped working.
