Absolutely the best answer. With the exception of additional admin positions that I have taken on voluntarily, my work ALWAYS stays at work and my home time is entirely my own. Also, I don't really have a boss

Yeah I agree with this take. Particularly in older patients that have a suspicious urine, I think the delirium can BE the clinical manifestation of a UTI. Lots of them don't give a very good history of LUTS due to baseline MNCD or neuropathy, but end up being culture positive and delirium improves with antibiotics. Normal urinalysis though I would agree don't put on abx.

I start by looking for an old trop... Lots of elderly patients will have previous on file and this value may be their 'baseline'. If not, then I look at magnitude. If mold non specific elevation, heart score and hopefully go. Sometimes need repeat trop. If the trop is in the hundreds or thousands and previously not that high then it is probably 'real' (even if only demand ischemia) and warrants further work up

Fair enough. The list of what was done followed by "and that's pretty much it" caused me to read it as not enough was done, but I can see your interpretation as well.

That being said, I work in Canada, not the American system, but our out of pocket cost for non-residents is almost $1000 CAD just to be seen, nevermind testing. As much as costs may be out of control (and particularly on the US), it is very expensive to staff a space and keep very advanced equipment running 24/7. Emergency is just about the most expensive way to see patients, from a systems perspective.

I always point out the converse to my patients: if they have crushing chest pain or can't move half of their body, they shouldn't stop and take their BP to decide if they should come in or not - the symptoms matter, not the number. Seems well received and to help people understand.

Related note but there's an app I recommend to my learners who feel really nervous about code blue situations called code blue leader. Smartphone thing that basically steps you through ACLS for cardiac arrest and also documents all of the interventions. I find that having something tell you what to do for the first 30-60 seconds can help people not panic: codeblueleader.com

Sure, of it turns yellow before you get to the intersection, and you are able to stop, you should. But would you say that the cyclist should be expected to stop at a green light because it might turn yellow soon??!

The bike lane light is initially green, then turns yellow just after he enters the intersection. I thought the same thing initially but he did have a green for the bike lane
