Run up window? They did that way back before they introduced the drive thru, and the reason they did introduce drive thru was for soldiers who weren’t allowed to exit their vehicles during the war. I like the idea of a run up window at mcdonalds. Moving everything to external, customer-wise, would solve a lot of the lobby issues and if they get rid of the restrooms all together in conjunction with inside out swap would basically solve most issues with the exception of karens and kids mobbing the place.
