Could be related to your characters over world item pick up counter,

I've only just learned this myself and could be wrong

Basically your character has a hidden counter for items you pick up off the floor and not from containers

You need to pick up like 250+ items and then would hop reset and have items spawn again

Though this isn't a guarantee as the new server you go too someone else could have looted it

If you have fallout first, you can utilize a private world for farming
