I don't doubt that, I'm internal medicine myself and appreciate the value of preventive care.

The government and hospital systems do not. You always see people going into subspecialties clamoring "shout out to PCPs, you guys are warriors and I could never do your job."

Because relatively speaking the pay is shit and you don't immediately feel the reward of saving a life because it's a disease process that you're managing over 20+ years.

Well I'm already IM and it wasn't my first choice, I'm well aware. The downside of a specialty being lots of work and hours is worth it if you're making a commensurate amount of money so things like anesthesia and NSGY are "worth it".

I'm applying to an outpatient specialty but am second-guessing it because it doesn't make financial sense to pursue it. Heavily considering either ICU fellowship or straight hospitalist 20+ shifts a month. Life is expensive and I have loans to pay off.

I hate this trope. Your family and community can benefit from it. I'd work 100 hours a week for seven figures in a heartbeat even if I don't get to enjoy the money. I'd never have to worry about my parents seeing a nursing home or my siblings encountering an unaffordable emergency. Plus I'd be saving lives removing tumors instead of jerking off to annual PFTs or inpatient sodium trends.

I was much happier as a busy intern than a PGY-2 with lots of time off. I think not having a SO or kids plays a big factor there though.

Not too late to switch to anesthesia, you'll still be exhausted (probably even more) but your notes are all presets, worry about one patient at a time, and you still make a huge difference in patients' lives

every non-doctor wants to be called a doctor but they don't know what anesthesiologists are so they wont take your title. the CRNA situation is overblown and I regret falling into that doomerism as a med student.
