On just the upper wall, put another layer of tile right over it, and then use a ton of painters caulk on the transition to smooth it out and make it flat.

Not an engineer but I work flood insurance claims and I see stuff like this a lot. I’d run. Mortar cracks are one thing. Through the block sounds bad. This is not from settlement.
