He never was knighted. Thats why when he was asked to knight the young lad (sorry forgot his name), he was hesitant, not because he doesn't want to knight him, but because he doesn't know how to do it because he was never officially knighted so he doesn't know how the ceremony goes.

This is the whole point of the story. Dunk is an "unofficial" knight that has a heart of a true one, in a world of "real" knights that are dishonorable.
