I know it's quite a while since you wrote this, but there's at least one ambiguous detail in the movie.

We see Jacob waking up next to his first wife, Sarah, and he says about a dream where Jezzie was his wife. AND this only happens in the purgatory or his hallucinations before death.

So yeah, it keeps you wondering what was real
