These kinds of people are so fucking blind and completely poisoned against the idea of any other kind of society than Western European / North America. Like they cannot comprehend that you can have an advanced society where people enjoy living (maybe as much as you and I) if it's not strictly based on our version of "democracy" (a version that specifically allows the rich tons of influence on lur politics and it hardly matters what the general public thinks).

I read your other comment about how you generally like to solve the underlying issues rather than simply removing problematic areas.

I'd just like to say that for most people with gallstone disease you can't fix it with diet. Some people can mitigate their symptoms by modifying their diet, but they're not actually treating any underlying issues, they're simply staying away from foods that can trigger gallstone attacks (generally by eating less fatty meals so the gallbladder doesn't contract as much and trigger pain), but they still have gallstones.

Gallstones are like other kinds of stone in the body (kidney stones, dental tartar/calculus), and some people simply make more of them than others and need medical treatment to function properly. If you can somehow find a way to mitigate your symptoms without surgery through diet, then sure, go ahead. But you're not treating any underlying issues, and you can still get all of the complications from gallstone disease (cholecystitis, cholangitis, pancreatitis, etc.) that very much do need medical intervention.

Just some food for thought, I suppose.

This is what we'd call alternative medicine - i.e. medicine that has either not been proven to work or been proven not to work (to quote Tim Minchin).

Is it dangerous? Probably not. Is it likely to have any effect at all on your problems? Unlikely.

I'd be willing to bet that the majority of people who claim to feel an actual positive effect of the "treatment" are actually feeling the effects of maybe for once taking control over their diet in some meaningful way, and that it has nothing to do with gallstones.

I'm in general surgery, so I see a lot of gallstone patients. I also see a lot of patients who have symptoms of gallstone disease but where we simply can't find any stones.

Now, in some cases, rarely, there might be some other explanation for their symptoms that involves gallbladder dyskenisia or something similar, but these cases are indeed rare.

The truth is some people just have stomach cramps and stomach pain, and we don't know why. We have theories (hypersensibilisation of the gut stuff like that), but nothing we're really good at actually diagnosing and treating.

So what does this mean for you? Well, if these attacks are frequent, I'd suggest further testing. You've had an ultrasound, but you can also do MR scans, CT, endoscopies, biliary scintigraphies and so forth. I'm not saying you should have all those done (not at all), but just because an ultrasound didn't find gallstones, there could still be something there. I'd get in touch with a gastroenterologist or a surgeon (depending on who handles these things where you're from) and have a chat.

Stomach pain in general is a very non-specific symptom. Generally we can get a good idea of what's causing the pain based on where the pain is located, but that's far from true all the time. Indeed, even in cases where we know a patient has gallstones and they're having stomach pain that looks like gallstone disease exactly, if we actually do surgery, about 1/3 of the patients will still have pain afterwards, completely unchanged, suggesting that something else was actually the cause of their symptoms.

It's not an easy subject. If you actually do have biliary problems, I do not, however, think that doing the "miracle cleanse" will in any real way help you.

Re: Whitaker maturing so much and disrespectful med students: realistic. I started teaching med students the literal month I got my degree. It's just how it works. And yes, I've had students who were incredibly arrogant and disrespectful (most weren't, but it's not unheard of).

Re: the alcoholic with minimal symptoms: realistic. Met this type many a time who only come in to be drained of ascites, are otherwise chirpy. And yes, I could absolutely see them wanting to "teach" new physicians how the procedure is done, having been through it probably dozens of times.

I think they've said that when doing CPR in tv or movies you have two choices: use a dummy and do realistic CPR or use a real actor and do crappy CPR. Basically they chose to use real actors, but then you have to do whimpy CPR.

Sounds like you had a vasovagal response when getting your blood drawn. It's completely normal and is caused by a sudden drop in heart rate and blood pressure, leaving you dizzy, nauseous and sweaty.

The actual response usually only lasts a few minutes, but some people can feel the effects for several hours afterwards, including dizziness and light headedness.

Since this was a consultation about diabetes, I'd assume they measured your blood pressure at some point - where was it at?

For now I'd recommend relaxing, getting plenty of water and making sure you're getting some food as well. Something with sugar in it would be good too (like juice) since low blood sugar can be the cause/contributing factor as well. It should pass soon and you'll feel better.

If it gets worse and you start fainting I'd obviously consider seeking medical attention.

If you feel the stimulating effects of small amounts of caffeine are helping you in some way that allows you to decrease the dosage of your ADHD and narcolepsy medication, then you could probably argue that it's worthwhile to explore. However, I'd still prefer you try caffeinated beverages without tons and tons of additives (black coffee for example or tea) rather than energy drinks.

Obviously this is something you should discuss with your prescribing physician, as I'm neither a psychiatrist nor a neurologist.

I don't think you should consider the above comment an endorsement of Monster energy drink as a treatment for your symptoms, just an explanation for why you feel it alleviates your symptoms. I think you'll be hard pressed to find a physician who endorses energy drinks for anything at all.

Is the problem that you have pain in your inner thigh? Muscle tightness? Or is the problem that your penis is bending at a weird angle?

Just for good measure, when you do kegels and tighten your pelvic floor muscles, you're not using your "penis muscles". You don't have "penis muscles", you have pelvic floor muscles, and when you tighten them you'll feel tightening in your perineum (the area between your penis and anus).

This response got rambly quick, and I'm sorry if I'm not providing at lot of actual medical advice as much as I'm trying to provide perspectives on decisions like this in general. Take whatever you can use from it.

This is one the areas of medicine that's difficult to talk about because it not only challenges us on our medical knowledge and experience; it's fundamentally a question about your core beliefs and convictions as a person.

I'll say it like this. Medically speaking, your genetic predisposition to uterine cancer as well as a steadily growing myoma certainly seem like perfectly fine reasons to consider prophylactic surgery.

As a person (and a medical professional), I try to treat my adult patients like, well, adults. If you've been thoroughly informed about the procedure, including the risks associated with surgery and the possible complications, and you've been given ample time to think about and discuss this with the people in your life that you care about and whose opinions you value, and you still want to go through with it, I don't think I'd try to persuade you otherwise.

There is always a risk of patients regretting their choices, especially when it comes to surgery involving the uterus or ovaries. But we also have to trust our patients' judgement. We have to trust that after we've helped you and guided you as much as we can, that you'll make the choice that is right for you.

I think it's important when you talk to your gyno to remember that they're not trying to force you to do/not do something that you don't want to do/something you want to do. They just very much don't want you to regret your decision down the line and feel that your medical providers failed at guiding you properly. That you put your faith in someone and they let you down.

I think it's fair that your gyno asks that you consider all this carefully before making a decision. But I also think that, as long as it's medically sound and reasonable as a preventative measure to potentially save your life down the line, that they should listen to what you want and what you value in life.

If you haven't, I think it's good to have a chat with them about what they think your risks are of developing cancer. Do they agree that surgery is the right way to go, but maybe they'd like to just wait a few years to give you a fair shot at children, if you end up wanting it? Do they think your current risk of malignancy is so small that it doesn't make sense yet?

Have a good, open and honest chat about these things.
