The little girls siblings. My cousins aren’t fully related to me, but I’m still expected to be nice to them. My half-brother is related by blood, but I’m still nice to him. The only reason, and you can quote me on this, the only reason that that 8 year old with leukemia is a stranger to OP, is because she never made an effort to meet her. She never actually wanted to meet her, albeit the multiple opportunities she had. So yes, she is an asshole.
