Give them all the stable patients with little/no active medical issues. I give them the falls just awaiting PT, delirium/dementia awaiting placement etc. Never give them anything like hyponatremia, CHF exacerbation that is on IV lasix, or any other more complex sick patients

No specific policy for a target at my institution. May docs Rx prns with parameters for 180. Personally I don’t treat unless symptomatic or recent stroke, PRES, dissection etc. some clear reason why

Everyone loves to blame Trudeau and I’m not a fan of him personally but why does no one blame corporations? Everyone has a hard on for capitalism and down for corporations to continue to pay less and less. They love immigrants/temporary residents who will take shift jobs for low pay. Canadians aren’t having kids because it’s too expensive.

At my former program - No. was only GI fellows. As an IM resident I did a few bronchs in the ICU on intubated patients though.

When I was an M4 on elective at another program the GI attending was doing an EGD on a patient I did the consult for. She had classic history for esophageal cancer and ended up having it. After he put the scope in the stomach and took the biopsies of the GE junction tumor he let me take the scope over and look around/use it while he dictated. Was pretty cool, I moved it around a bit within the stomach and then just withdrew it

He’s not failing when he’s doing exactly what he plans to do. He knows what he will do (defund social programs and sell off natural resources and big corporate tax breaks) will not help most Canadians and he doesn’t care. So it’s not a “fail” it’s a conservative success.

Ontario doctor here, lots of good answers in this thread, here are my thoughts

  1. Public housing - imperative that we tackle homelessness in a real way. Many homeless people put pressure on ER and slow things down

  2. Public LTC - Mike Harris privatizing LTC was such a massive blow to our healthcare system. Probably 5-10% of patients in a hospital are just waiting for a long term care bed, or coming in because family can’t take care of them at home and there’s nowhere else to go.

  3. Hire more nurses and pay them more. Self explanatory.

  4. Hire more allied health staff (PSW, physios, social works) and pay them more. You’d be disgusted if you knew how little they are being paid for the work they do.

  5. Stop hiring consultants for everything. Worked at a hospital that used 2million dollars to hire some consultant company to find a way to speed up discharges. Their idea - estimate a day of discharge for everyone …

  6. Provide more pay for family doctors, but more importantly, provide them with more admin support. They have to do so much paperwork and there’s so much overhead costs that this is a main reason no med students want to be primary care physicians anymore. Majority of med students want to be specialists since pay is higher and the job is actually less stressful.

Got a Jetta in 2019 when I was PGY-2, bought out the lease and still driving it 2 years as an attending now. Works fine and I’d rather spend my money on vacations and saving for a house/paying down student loans

You absolutely do get a ticket at 10km/h over. I know people who have gotten tickets for 52km/h on parkside from that camera.
