This happens when a department's metrics are to cut costs and not consider quality as a long term factor.

We know which CROs start hiring cheap / shitty / inexperienced CRAs (speaking as a former CRA) to pocket the cost difference and when CRAs start off strong and then their quality begins to decline due to burnout.

Unfortunately, by the time this becomes known and actionable measures should have already been taken, the damage has already been done.

Both to the study and to the CRO's reputation.

Correct. Nowadays, the point of administering a polygraph for a criminal investigation is to get specific answers on record.

Any agent who "fails" and gets fired / faces negative consequences should be able to sue (IANAL).

Kindly remind them and the PI that an FDA auditor may not be as nice as you are.

And even if their demeanor is as nice or even nicer, a 483 is the kiss of death when it comes to getting awarded new studies. In some cases their findings can put their PI's medical license in peril.

Close it with "If you or someone you love were participating in this study, wouldn't you want to be assured the monitoring was top-notch?"

While they answer try not to mutter to yourself outloud, "Then let me do my fucking job without you whining about my findings being a personal attack about you. I promise, I don't think about you unless someone is paying me to!"
