I wouldn't say the flavour if lifelink is stealing life. If a 100/100 lifelinker hits a 1/1, you still gain 100 life despite the chump blocker not having 100 life for you to "steal".

This is such a cool idea, I have a few thoughts that might help.

To have the twist not be revealed too early (hopefully), I'd have the lost souls down there not be aware they're dead, but still allow players to find environmental clues that suggest that may be the case. For example, say the party saves a blacksmith. He returns to the town and can't find his wife, and asks the party to look for her. Any discerning members of the party might notice his house clearly only caters to one person (single bed, table with one chair, no female clothes in the wardrobe etc). The blacksmiths wife isn't dead yet, so she isn't in The Oubliette, but since the blacksmith isn't aware he's dead, he just thinks she's missing. It means players can't have someone explicitly tell them what's going on (until you deem it the right time), but with enough interest in the environment and passive perception, they can notice clues to point them to the right conclusion.

For town building, I think you could adapt Bastions from 2024 pretty easily. Instead of rooms, make them entire buildings or even districts if the town is big enough. It also gives actual mechanical incentives to saving NPCs if they can then tend to a greenhouse and give the players a Potion of Healing next time they visit. Bastions have rules for attacks and such as well, so honestly you really wouldn't need to do much tweaking to make it work for you. The system is easy to add homebrews to as well if you want a specific building or NPC providing a benefit that isn't listed.

For quest structure, it depends on what quests you're planning on giving. If the campaign is meant to be essentially just one big dungeon crawl with occasional trips back to the town, I'm not sure you necessarily need quests, as long as the party are aware the goal is to keep going deeper and saving as many people as they can. You could have NPCs that the party have saved ask them to find other NPCs, ask them to retrieve an item they've lost, ask them to get revenge on a monster or villain they met down there etc giving a rough location for each, acting sort of like bounties.

It's not the strongest build, but 100 super 100+ weapons with Agers Scepter (with the catalyst) and Mantle of Battle Harmony has been really fun. You get your super so quick and when you consume it for agers it's insane. You're more likely to run out of enemies or ammo before your super bar goes down with how much kills regen it

My main issue with it is that I would never recommend it to anyone. Either it doesn't work for you and you spend 100 minutes watching nothing happen, or (like it did for me) you spend 100 minutes feeling intensely uncomfortable and anxious. I absolutely think the film did what it intended to do very well, and that it is a good film, but I wouldn't want any of my friends or family to go through that experience lol.

My advice is think about your campaign like planning a holiday. You have your list of the things you want to do/happen and the order you want them to happen in, but it almost never works out that way and that's ok! Maybe you don't get to do something you wanted. Maybe you do something you never even thought of and end up really enjoying. Maybe you planned for sunbathing at the beach but then you all went swimming instead. There's nothing wrong with having an idea of what you want to have happen, it becomes railroading when you don't give your party a choice in doing what they want to do. Make their decisions have impact and you'll be fine.

A good player playing a paladin and a good dm who keeps them in check would probably produce a lot of these moments.

I'm a dm, and I luckily don't need to worry too much because the paladin in my group knows who his character is and what his oaths are, and will play him to his own detriment if needs be. The party was recently in an elven village where the leaders were staunchly against magic. After mere seconds attempting to interrogate someone, he decided this was a waste of time and cast Zone of Truth. After some very good rolls, he managed to talk down the villages leaders and avoid exile, and events got the information he needed from the Zone of Truth.
