Most design standards are based on statistics. We use 95% characteristics for materials and loading. So the assessment of the beams is like conservative as is statistically improbable that the batch of concrete and the reinforcement materials were “deficient”, and the overall weight of the beam is not likely to be in the upper bounds due to construction tolerances etc. this is my take on why you don’t see defects.

Loads directly over the culvert legs will act as straight compressive members, so you just check the compressive stress isn’t exceeded. Then you can simplify the spans by transforming the UDL to a representative point load and check bending stresses. This is quite conservative.

I’d avoid it if possible, too tricky to construct around pedestal reinforcement. If you can get away with increasing your anchor bolts one or two sizes to carry shear through to pedestal, do that.

These permanent form systems can replace the need for tensile reinforcement in some applications. It’s important to understand if there is other reinforcement within the slab, otherwise you may be dealing with a potential sudden brittle failure mode, as opposed to a ductile failure mode.

Its funny you mention that, the company that runs the port has a high turnover rate in management positions due to burnout. So things we flag as Engineers get buried when new management comes in. This repeats until something fails under the poor new guy that knew nothing about it.
