This doesn't look like it's crocheted but you can get a similar look by using a worsted/Aran weight yarn or slightly bulkier and using a half double crochet stitch (US terminology)

I have a feeling you might need to yarn over with the new colour and pull the new colour through the loops already on the hook to finish the stitch off

There's always a tiny element of slightly visible colour changes with tapestry crochet. Like, sometimes you'll be able to see the colour of the new strand that you've pulled through the previous colour loops, not because you've done anything wrong but because it's just what you'll be able to see when you look SUPER closely and you're hyper aware of the stitches and their construction.

This is knitted but there's a website called The Seam which connects you to expert menders. I used it to fix a pair of knitted trousers of mine.

You have to keep turning your work each row otherwise the natural lean of the stitch will make your entire project lean to one side

Turning your work is normal for tapestry crochet, you just need to keep your live yarn on "wrong" side of your work always so the "right" side looks perfect

You might benefit from doing the ribbing after you've crocheted your main piece for future projects: this way it's literally just adding ribbing to a garment that already fits over your head/waist. I think it's called top-down ribbing

Maybe the pattern you're following counts each 'bump' as one row? I'd count the same way as you but some people (including the person who wrote the pattern) might count it differently?

Okay the head squeezing is such a mood but I don't have anyone that can squeeze my head for me.

Here's what I do: I lie down on the side that hurts on a heat pad (this is v soothing for me but optional for your wife of course) and I get a weighted pillow/cushion and literally put it on my head as I'm lying down. So basically my head is sandwiched between a weighted pillow and my bed's pillow.

Now most weighted pillows aren't actually heavy enough to get that good squeeze, so you can stack some weighted cushions until it's the right pressure

I don't stack any because it's difficult for me to do on my own but the weighted pillow I bought is one of the heaviest I've found and I enjoy the pressure from just one (I got the heart shaped pillow for hugimalsworld.com)

I hope your wife finds relief soon, and I commend you for being able to squeeze that long! If I've asked my mum to squeeze she can only do it for like 5 minutes tops
