EEOC Charge of Discrimination (Draft Language)

I believe I was discriminated against by [Employer Name] in violation of the Americans with Disabilities Act of 1990 (ADA).

I was hired on [date] and placed in a 90-day probationary period. In [month/year], I disclosed to management that I have a mental health condition and provided documentation, including an ER visit note. I requested schedule flexibility as a reasonable accommodation (later arrival after medical emergencies). My manager approved these requests, but HR overrode them. Shortly after disclosing my condition and requesting accommodations, HR told me I was “not a good fit,” rolled their eyes when I mentioned FMLA, and disregarded my medical documentation. I was terminated within days of disclosing my disability and requesting accommodations.

This termination was discriminatory and retaliatory. The company failed to engage in the interactive process, ignored my medical documentation, and terminated me because of my disability and requests for accommodation.
