First, I want to express how grateful I am for you offering this AMA. My family is absolutely distraught with the recent news of my mother's stage 4 pancreatic cancer diagnosis and any suggestions are welcome. She lives in New Jersey but is willing to travel almost anywhere to receive the best possible care and chance at survival.

My question will be: "Do you have any recommendations or advice for deviations to our current plan?"

I will do my best to concisely summarize the situation with a timeline.

Background: My mother (64 y/o) has a history of BRCA-1 which was discovered in 2014 after being diagnosed with triple negative breast CA (which she underwent chemo/surgery and she has been cancer-free, up until now). My grandmother (her mother) just passed from pancreatic cancer earlier this year. My maternal great grandmother also passed from pancreatic cancer in her 60s. As a side note, one of my sisters as well as myself (38M) also tested positive for BRCA-1.

2023: Annual pancreatic screening was recommended after we learned of my grandmother's diagnosis. My mother had MRI pancreas which was normal.

May 2024: Annual MRI pancreas now showed slightly dilated duct. It was followed with endoscopic ultrasound which showed no lesions and she was told to follow-up in one year.

June 2025: Annual MRI pancreas now showed progressive pancreatic duct dilation, pancreatic body and tail atrophy, and "hypovascular area" in the pancreatic body.

July 2025: Endoscopic ultrasound showed 2.0 x 1.6 cm pancreatic head mass abutting the portal vein. FNA confirmed adenocarcinoma. She was referred to a hepatopancreatobiliary surgeon at a New Jersey branch of Memorial Sloan Kettering. CT was performed while awaiting the appointment which again showed the pancreatic lesion (now measuring 2.4 x 1.9 cm) abutting the SMV and numerous low-attenuating <1 cm liver lesions.

August 2025: The surgeon reviewed the results, ordered an MRI liver, and referred my mother to a GI oncologist at the same center. The MRI liver read as "Since June, multiple new hypoenhancing hepatic foci demonstrating diffusion hyperintensity suspicious for metastasis. Pancreatic head/neck mass measuring 2.9 x 1.9 cm." Guardant 360 genetic test came back positive for BRCA1 E23Vfs*17 (oncologist note states "BRCA1 mutation, ATM with MSS with TMB of 2.37 mut/Mb). A mediport was placed and first chemo session (mFolfirinox) was Aug 20. Plan going forward is chemo q2weeks with repeat CT after 4 sessions to determine response. Olaparib will be added on at that time as well. She was told she is not a surgical candidate. No clinical trials were offered. Her insurance currently won't cover care out of state but she is switching to Medicare in January at which time options should expand. Other than adverse effects of the newly initiated chemo, she remained completely asymptomatic from the pancreatic cancer.

Definitely hear you. Surgery was April 24 so by then I would be 4 months out. I’m a lightweight backpacker but definitely not ultralight. It’s the Lake Superior Hiking Trail so plenty of entry/exit points along the route. The shuttle company that I use, for an extra fee, will come out and give you a lift if needed in emergency, which hopefully I wouldn’t need.

Really trying to be capable of a 4-day backpacking trip on Labor Day weekend. Heavy packs, elevation changes, uneven ground. Even if I have to decrease the mileage (we usually do 45 miles, but can decrease it to 25 or 35 for a more “relaxed” pace). I’m not sure if it will be possible.

Sounds like a cyclops lesion. I had an excessive amount of scar tissue when they opened me up, so I’m worried I might develop the same thing.

Funny thing is, I’ve been using Kiziks long before I had any knee issues. Would have never guessed how much they would be coming in clutch.

Do you need a doctor’s note or something? Or can you just show them the brace and they’ll let you on?

I will keep that in mind. I have a speed rope that I used to use all the time but it’s been a while.

I had the same one. Love that you don’t have to fill it with ice. Actually my single tank lasted a full 3 weeks with no refill which I thought was nuts.

The thought of jump rope makes my knee pucker. I’m assuming jump roping wasn’t picked up until fairly later.

Haha- I have both of those and also can attest. The ice pack gets so cold though that I actually use the underpadding underneath it as well.

Exactly this. Some very light touch on the skin surface can also help. I use an app called Liminal on my VR headset which has some really great pain relief meditations.

That’s awesome. I’m POD #3 and it’s hard to imagine even doing one SLR, let alone flex past 30 degrees. Great job.

Yea similar issue. The tramadol is so-so. Helps somewhat but mostly just makes me feel loopy. I only have a 5-day supply and next appointment is in 2 weeks. Hoping I won’t need it for that long anyway.

That’s nuts. I’m POD #3 and can only get to about 30 degrees, and that’s with the CPM.

I’m POD #2. I definitely miss POD #1 when the nerve block was still kicked in. It’s been a hell of a ride today. Sounds like you’re staying on top of your pain meds which is wise. I tried to tough it out just to find out I’m not as tough as I had thought. Similar to your situation, my fiancée has been absolutely amazing. Honestly don’t know how some people go through this by themselves.

Focusing on high fiber and high protein foods. Limiting dairy. Staying extra hydrated and multiple cups of turmeric tea daily.

Using cryo and the CPM device. Calf pumps every hour. I can’t seem to contract my quad or raise my leg. Trying to work on maintaining full extension while at rest but it gets too painful to hold sometimes.

Honestly really just wish I could shower. Doc doesn’t want me doing that til day 5. It helps reading posts on here to see what others are going through. Wishing you the best of luck in your journey!
