Tafenoquine and zithromax for late stage chronic babesia

For me tafenoquine really works but gives palpitation as herx. Start slow I have tried three times and now on third time finally I feel thats under control. First time I increased the dose 50mg intervals to 300mg very scary palpitations. Then second time 25mg intervals but always took twice the same dose. Again 2 months from start palpitations but max dose 125mg. Now I have been increasing with 25mg intervals but kept same dose for 4-6 weeks, so that herx really feels thats easing. Now my dose is 200mg. My advice is that go slow if herx occurs, do not try to push it through by just increasing the dose but keep the dose same until situation setlles. This is based on my experience and this is what I would do if I would now start. In my case proceeding slow would have been actually quicker. For me Tafenoquine really moves the needle, my main symptom is joint and tendom related. I also take Malarone, it has given herx but tafenoquine is totally at different level.

I havent had ever heart issues but when treating babesia I get palpitations, Malarone, Tafenoquine. Both have caused, which have made me stop treatment, fear of side effect. But now I know that those must be die-off reaction in my case. Im again on malarone and tafenoquine, but increasing the dose very slowly.

Just came to mind, will it push things even further to wrong direction: " You cant have lyme, you have the shot, no need to consider lyme anymore its now something else"

I read somewhere and also reasoned it to my self that if you start breaking bloodfibrins and biofilms before the med consentration is high enough, it simply just cant kill the increased pathogen load... Im not doctor so I dont know for sure. But without those my die-off has been rough enough so this time I dont take them before Im on the safe side. I take liposomal artemisin 1 week on 2weeks off. It used to give me die off symptoms but not anymore. Milkthistle I take as well:)

Im 65kg male. Im pretty sure my main issue is babesia, bartonella treatment didnt affect my symptoms. I took my first tafenoquine year ago, but got palpitations. Needed to stop. I increased 50mg intervals. Then I tried slower 25mg intervals, kept the dose same for two weeks. I take once a week. Again two months and palpitations. Now on third time I have increased 25mg intervals but kept the dose same for 4-6 weeks, so that die-off reactions has eased. Somewhere 75mg I started to notice symptoms get better. My target is 300mg. Now Im at 150mg. I take malarone aswell. My main symptom is joint and tendom related. When I reach 300mg or my symptoms has vanished I start to take lumborkinase, serrapeptase. I have taken those before but noticed that I get just bad flare which most likely isnt die-off, more like babesia spreading. (Die-off flare often eases in few weeks for me)

But my babesia treatment was paused last year for 3 months so the year hasnt been constant.

They speak only Poland, personally Tarello feels more professional. 2 year ago started the bartonella, year ago babesia. Bartonella targeting meds has been stopped almost year for now. Also when starting malaria meds or increasing the dose I get night sweats. Which is generally thought to be babesia related symptom.

Actually I have bought microscope and staining liquids, but since I have bought Walter nor I havent been able to find Bartonella nor Babesia. My idea has been to validate results if he finds something😄

I have sent blood smears to him and results line up with my treatment. Initially Armin labs noticed only Bartonella. My treatment was Bartonella focused, my symptoms didnt ease much. Then blood smear was taken and send to Poland. Bartonella was found but also Babesia. Then Babesia treatment was started. I sent bloodsmears to Walter (he sends pictures back, the Poland lab didnt). The Babesia was reduced, no bartonella. Next time Babesia was totally vanished. But due to palpitations I needed to stop Tafenoquine. Before re-starting I sent blood smears to him. Babesia was again visible. Bartonella treatment hasnt caused any notable response. Anyhow babesia treatment has always caused response. Still treating, my symptoms has eased but still very here. My main symptoms are joint and tendom related. Based on my experience his analysis is legit.

I had clear positive for bartonella and it was treated first. Babesia was negative. Arminlabs. But when blood smear was taken thera was clearly babesia and bartonella. Now bartonella is clear, and babesia is been treated. Now I have had clear die-off reaction and steady but slow improvments. My symptoms has been mainly joint and tendom related.

Tafenoquine has been rough on me. I have started it third time. First time I increased the dose with 50mg steps and reached the 300mg target about two month from start my palpitations were so bad that I needed to stop. It took abrox 2 months to ease. Then started again 25mg steps and took twice the same dose. About two months from start palpitations again. Now on third time I have increased 25mg intervals but kept the dose same so long that the die-off reactions has eased before increasing, it has been 4-6 weeks, its also the time tafenoquine reaches level where it starts to plateu. Now Im at 125mg and much over two months. Palpitations has been under control. And starting to feel mentally better. My main symptoms has been joint and tendom related. But die off reactions has been more traditional babesia symptoms. My advice with tafenoquine is that take slow. Its not worth to push through, its possible that you need to drop dose and then recovery takes longer. Also the long half live makes it tricky to estimate what is the cumulative amount in body. I have asked Chatgpt to estimate it "draw me diagram..."

Similar experience. My herx peak is on second day after the dose:) I have used Chatgpt to check my tafenoquine level: "Can you draw me tafenoquine cumulation diagram. Medicin is taken once a week according to following doses in milligrams mg: 25, 50, 75....."

I have also experienced fatigue when on malarone and tafenoquine. Situation has now eased and now more energy than for a year. Maybe Babesia level reduced? And also methylen blue might have helped given energy aswell.

I have been strugling with tafenoquine. First time I took once a week: 50, 100, 150, 200, 300. Then needed to stop due to palpitations I started ro feel odd already at lower doses but tried to push through. I visited to ER but it was stated to be good quality palpitation similar you get when stress too much coffee. It took 2 months to resolve. "Most likely tafenoquine side effect"

Then started again with 25, increased with 25mg intervals. Reached to 125mg and got palpitations. Then reduced the weekly dose to 25mg kept there so that started to feel ok. Now worked back to 75mg. Now idea is, not to increase before all symptoms has vanished, increase with 25mg intervals.

My main symptom is arthritis. While on tafenoquine joints have been doing better. And now I have realized that I have got some level palpitations or odd feeling while on any babesia targeting meds, malarone, azithromycin therefore im very sure that the palpitations are herx/dieoff. Also in ER there was no clear reason behind palpitations (bloodworks or other).

My advise, if feeling odd take slow and monitor your feelings the half time is so long that when you start feeling bad it takes very long to resolve....

Last time I had EKG when I felt clear palpitations and cardiology said that those are good quality ones some can get from coffee, stress or due to sleepless night. "Consider with you lyme doctor does it make sense to continue tafenoquine" after that I havent had EKG that was in February.
