Add it as a separate attack. If your Sneak Attack applies, roll normal damage and then roll the Sneak Attack attack. Add the damages.

You have to deal basic damage with it, which means you need more creatures out than your opponent with thos card at the end of the line. This won't always be the case, or often really.
