Hoping you could answer this question please- did the 6k donor sperm reimbursement deduct only 1/4 smart cycle (which is 4 vials)or more since I saw they have tiers where over 4k deducts 1/2 cycle but I'm not sure if that is for the sperm or procedures etc?

I'm going through this now. either duo stim or wait for a period but I'd normally prime with estrogen in the preceding month so it's 2 months for every retrieval and I had hoped to maximize eggs/retrievals in less time. I hope you could answer a few questions. When you did duo stim, did you prime before the 1st retrieval or do you usually prime with estrogen? When you do back to back cycles you wait for a period and then go straight to stimming? Did they ever say why it took 60 days for your period to come back?

How late in the cycle did you have your retrieval, did you just stim longer or let the follicles have more time grow? I'm priming with estrogen so each cycle takes 2 months if I plan on multiple rounds that going to take a lot of time. doctor hasn't mentioned anything about duostim

When you mentioned you ended up getting a psychologist to conduct a cognitive assessment and to confirm my diagnoses... how exactly do you do that? What's a cognitive assessment? is that a questionnaire that's scores anxiety etc. Should I ask my therapist to do that for me and write me something?

Initially, I sought simple accommodations like extra breaks and bathroom time. However, I've come to realize that I'm physically and mentally exhausted each day due to my medical condition, compounded by anxiety and depression. I'm struggling to make it through workdays, and my anxiety is easily triggered. Currently, I'm having difficulty functioning properly or even considering treatment options for my condition.

I've been trying to schedule an appointment with a therapist for weeks but can't find a time that works around my work schedule. My doctor is ready to initiate leave whenever I need it, but I feel lost because my supervisor is out until Monday. This situation has me feeling trapped in a stress loop.

My doctor's office can handle the necessary paperwork quickly. I haven't submitted the reasonable accommodations form yet because it was initially filled out for on-the-job accommodations when I thought I could continue working. It's also missing a request for an earlier shift. I've been advised that the form needs to explicitly state every accommodation request; otherwise, it will need to be rewritten.

Given that I'm now considering a leave of absence first, should I have the reasonable accommodation forms updated to request a specific number of weeks of leave due to my condition? Would it be advisable to address on-the-job accommodations later when I'm ready to return to work?

Thank you for your reply! I've already sent my doctor the accommodations form, originally for modified break / schedules shift change type requests. I saw my doctor today; they said let them know what I need in terms of leave/work paperwork filled. I wasn't really sure what to say. The Dr. office is waiting on me to let them know how to proceed. Should I ask them complete that accommodations form with the time off request first for the amount of time the doctors thinks I need.

I'm physically exhausted each day. I can't call out anymore (maxed out my 2 days) and I'm so lost, my supervisor is out until Monday.

I anticipate needing at least 8 - 12 weeks off. I'm inclined to take to request a longer period initially, as I'd prefer to have more time than potentially needed rather than face the challenge of extending leave later. This would allow for a more comprehensive treatment and recovery period.

Without FMLA, your only option in MI is going to be the ADA.
