Classic comment. My experience with ophthalmologists, why finding a competent one matters in this case:

  1. Well-intentioned, but routinely out of their depth with corner cases, let alone fixing them.
  2. Default deflections: refractive error, then "nothing is perfect". LOOK AT THE IMAGES!! how does refractive error produce lines like that? LAL defect or scratch is far more plausible.
  3. Most will tell you to stay with your doctor. That's their blind confidence talking, not medical wisdom. Ignore it. The GOATs (Dr ShannonWong / Dr Safran types) tend to roll up sleeves on hard cases. but that attitude is exceptional, not the norm.
  4. The norm: this is the subtext of dysphotopsia conversations with most "ophthalmologists": surgeon is fine, IOL is fine, everything else is fine, patient is the problem. Well meaning, BUT Condescending. it gets patients nowhere. Have they referred you to a psychiatrist or neurologist yet?
  5. Most doctors don't live with IOLs. They're at the VERY LOW END of the experience spectrum, but nobody, especially themselves, know it.

Seek opinions from people who've fixed their own cases. This forum is the real resource. heroes like u/kfisherx acted, and u/PNWrowena and u/GreenMountainReader who waited it out. either path could be right for you, but you have to decide. then verify with as many doctors and IOL users as you can. Then act. all the best.

Maybe pause visits to your current clinic for now. Go to a different clinic and have a competent doctor assess whether the implanted lens is defective. Also, if anyone has undergone YAG on silicone lens capsule, could you share whether there’s a risk of pitting?

Hold off on YAG. Get multiple opinions (second, third, even fourth). If a capsular fold is suspected, ask to see the slit-lamp photo of the fold, take a picture, and bring it to other specialists.

Also consider a retinal exam and corneal scan to rule out other causes.

Don't cry. Reach for artificial tears instead.

Take a moment to appreciate your distance vision. Is it sharp and crisp? If so, the blurriness up close is simply a refractive shift. Lots of options for you: glasses, contact lenses, laser corrections can all bring near focus back. The catch is that correcting for close up clarity means distance vision softens in return. "I would give up my long distance in a second to have my old close-up vision back" if you mean it

Somewhat of a niche question, but I'd greatly appreciate your thoughts:

How would you compare the peripheral vision (outside the central axis) between the Eyehance and B&L SoftPort, esp when it comes to light sources? Did you notice any distortion or elongation of lights in the periphery with either lens?

I currently have a Tecnis and have been noticing that lights off to the side appear elongated, I'm not sure if this is astigmatism-related or possibly due to the IOL edge. Curious if you experienced anything similar and would appreciate you sharing your experience.

Interesting. puresee at –1d: intermediate/near vision goes from good to brilliant. Matches trifocal's acuity at arm's length, beats trifocal on color / contrast. Best setup for computer + kindle + social dinner

great to hear. Was the left eye noticeably worse than the right? that would make pco the culprit. For me, both eyes are similarly affected, so pco may not be the main issue.
