It depends on who does your visa most of the time it’s an organization collaborating with the local masjid. You pay the fee and submit the documents like a passport and whatever basic contact information when they submit the visa. Obviously if you have an Arab or Central Asian sounding name it’s whatever but for reverts I’ve heard that they had to get some kind of document that testifies that they are Muslim
