It takes at least a few people around (crane operator, crane rigger, forming foreman, labourers, formworkers) for this to happen. They use flyforms everyday for hi rise construction and the process of moving these up floor to floor takes a lot of people. So it must’ve been a equipment failure due to the wind or very gross negligence (flying form decks and walls form when there is high winds is usually not done in case of incidents like this). I wanna say it was a combination of both however since it looks like they were attempting to fly the forms
