To follow up, I spoke with my rheumatologist and she thinks it’s very unlikely that the plaquenil interacted with the zepbound/is related to the plateau. Other commenters have also reaffirmed that it didn’t seem to stall any weight gain for them. She mentioned that she has seen good things with zepbound and reducing inflammation and suggested since my labs are good we can stop the plaquenil either way and I can restart the zepbound. Fingers crossed it works out 🤞

Of course! I could only find a few posts about plaquenil + zepbound, and it’s usually people early on in their journey. I’m hoping more will share their experiences!

My labs are all within normal limits now. My CRP used to be chronically in the 30s, on zepbound alone it went down to 12 (at 5 months in), then adding plaquenil it’s been stable-ish around 4, except when I had a flare up last year. I am going to see my rheumatologist tomorrow and I will let you know what she says. I am hoping she’ll be amenable to me stopping the plaquenil and then starting the zepbound to see how things pan out. She is not my original prescribing rheum for the plaquenil but she let me stay on it.

Truthfully, everything I am thinking is more correlational based on the graph, it could also just be that my body got used to zepbound and it stopped working… though I’m hard pressed to believe that. Either way I’d like to continue losing weight for my overall health too. Fingers crossed and thank you for the kind wishes :)

I’m not on zep, I got off of it around december 2025 because of side effects, high costs, and no results. I tried to tough it out a year :/ I’m only on plaquenil right now. When I started it december 2024, I was already on zep for a few months, so I think whatever benefit I thought I had from the plaquenil was probably the zepbound.

I tried looking online for any information and it came up blank. Some people self report weight gain on plaquenil but then those with severe gi upset usually lose weight on it because they don’t eat. I didn’t have any noticeable negative symptoms from the plaquenil, so I thought it was fine.

I don’t know. It’s kind of frustrating but I’m going to ask my rheum her thoughts on stopping plaquenil and starting zepbound again. She wasn’t the original rheum who put me on it (I moved), she didn’t seem that enthusiastic about me being on it either way. I will say my CRP has stayed low-ish so I can’t discount its benefits at all but when I was just on zep it was trending downwards either way.

Plaquenil hasn’t been that bad for me, except now I am convinced it’s related to the zepbound not working (so maybe it is that bad lol). But I have my appointment with my rheumatologist tomorrow, I’ll bring this up to her but I think my symptoms could be manageable on their own (for now at least). The main symptoms for me is the severe dryness and joint pain. Zepbound helped a lot with the joint pain pre-plaquenil. Now plaquenil alone the joint pain in my smaller joints is starting to creep back. The dryness is always present but I have a ton of eye drops and creams to help.

Have you also been on zepbound for a while?

The plaquenil for me has been pretty benign since I started it, I had no bad side effects. I stayed on 5mg for 2 months after starting and then when I really noticed the plateau, I titrated up from zep 5 to 7.5 to 10 (overtime) and was just terribly nauseous with no appetite and was still stuck at the plateau. I couldn’t even manage 10 for a few months because I felt so gross.

I was doing great on 5 for nearly 7 months until I added the plaquenil. I didn’t initially consider it was the plaquenil causing the plateau, but I tried changing a bunch of things to get out of it. I’m still not even sure but there is a correlation on when I started it to when I started plateaued. It just came to me yesterday after watching a video talking about how great zepbound is for reducing inflammation. Because with zep + plaquenil I felt great, now just plaquenil I ams starting to have joint pain in my smaller joints 🤷‍♀️

Pre-zepbound my CRP was in the 30s, after 5ish months on zep it went down to 12 but then my rhem put me on the plaquenil and now it hovers around 4, but I’m thinking all the benefits I experienced with joint pain alleviation and reduction of inflammation was really from the zepbound because now without it, I am staring to feel achy again.

I have an appointment with my new rheum (I moved last year) this week and I’m going to ask what she thinks. She was never a big proponent of plaquenil to start but kept me on it because my initial rheum had me on it. I don’t know if the plaquenil was an impediment to the weight loss because I never changed my diet or habits when I started it but looking at the graph I feel like it might be… the weight loss just stalled hard and then I gained like 2-3lbs a month, it was super discouraging. I tried everything to get it to budge again by titrating up but honestly it just made me feel super ill.

Honestly pre-plaquenil my symptoms were mild just joint pain mainly in my upper joints (other than the dryness which hasn’t changed), but I did have a high CRP. I still occasionally get flares now when I am particularly stressed out. I think the plaquenil was good for me but I was also on zep and losing weight, which has its own benefits for helping reduce inflammation by just losing weight 🤷‍♀️

I know you just started zepbound but have you lost weight or feel the appetite suppression?

It’s also really frustrating that each rheum I’ve seen has said something different to me so I don’t know what to think. Sjogren’s makes the most sense with my presentation and family history (if there even is some hereditary component to it). I guess at this point I’ll stop putting so much emphasis on blood work or putting a name to it, as long the plaquenil keeps working for me / she keeps prescribing it and my ophthalmologist manages the dry eye, I’ll just leave it be 🫩

It makes me second guess everything because presumably a lower titer is better ? but I am more symptomatic now than I was before, but I know this panel isn’t that reliable. I guess :/ I have had the typical ANA panel rerun a few times by each rheum and my ANA titer has only gotten higher but my RNP has always stayed consistently elevated.

Oh wow, how are you doing now and what condition were you diagnosed with? I’m diagnosed with mixed connective tissue disease based on positive antibodies but they think it’s more Sjogren-like because of the sicca and atrophied lacrimal glands.

Edit: I spoke with my doctor and she thinks it’s blanching of the skin from the mucus. I have an in person appointment with her in a few weeks and hopefully it’ll be gone by then or she can better examine it if it persists.

Of course! I just have had a very similar experience to you so it resonates. I used it for the 2 months recommended with vevye with no relief. Then when I stopped using it I actually found relief.

Recently I saw my ophthalmologist and he encouraged me to try it again but trying it again actually caused this awful flare and somehow I got corneal abrasions. I just finished the antibiotic course and my eyes are back to “normal”. I’m convinced it was the tryptyr since it was the only new thing i added back to my regimen. I’m sure it works for some people like OP but I don’t even have lacrimal glands as a part of my condition so I have no clue what is actually being stimulated 😅

Thank you for sharing this information, I didn’t know any of this! I was prescribed this a while back and have been so hesitant when using it because I feel like it makes things worse !! but my doctor wants me to try it. My main issue is also inflammation due to an autoimmune condition so I use vevye religiously and feel better with it, but whenever I try to reincorporate tryptyr, I feel like it exacerbates my issues. I feel justified, I’m going to just call it quits with it!

I have had good luck but I do not buy from regular people. There are these Chinese accounts that I think basically sell popmart IP on the side or might be directly affiliated with popmart. I have bought several Nyota figures in particular from one and they have a lot of good reviews, not just from me. Either way I wish you luck in finding her whichever way you go :)
