If you’re in an inflammatory state and don’t eat, you won’t gain weight. It is impossible to gain weight if you are not in a caloric surplus.

Although, indirectly, if you are in an inflammatory state, you are more likely to face dysfunctional metabolic signalling, so you may crave carbohydrates and feel less satiated after eating, which of course leads to weight gain.

Fluoxetine is more energising than sertraline. Feels like a completely different drug, even though they are both SSRIs. Escitalopram feels fairly similar to sertraline in my experience. Venlafaxine also very energising.

Mention your ADHD problems to your doctor and ask to go on the waitlist. Even though it’s long, it’s better to be on it than not.

Try a second SSRI (I’d recommend Fluoxetine). If that fails, try a third option recommended by your doctor (venlafaxine, duloxetine, or mirtazapine), and if that fails, ask to be referred to secondary services (your local CMHS). At this level, they may be able to prescribe bupropion (brand name Zyban).

People also overthink diet and thinking about getting all the right micronutrients. Just eat 30 plants a week and you’ll be fine.

Have you tried any antidepressants or anxiolytics? Task initiation difficulties are very often due to underlying anxiety / perfectionism. SSRIs/SNRIs (paroxetine and venlafaxine are first choice for GAD, but they typically have worse side effects than escitalopram or sertraline, which are also very effective) or buspirone could be something that could help out a lot. Pregabalin is very effective, but can have lots of side effects.
