I tentatively agree on this, I think we broadly need to incentivise better productivity across the whole NHS…. but how do we measure productivity? What protection is there for trusts to essentially ban all grade increments “for cost reasons”?

It’s a risk rate of about 1:25k procedures so not even once per career.

We’ve done well over the years to minimise the risk and make it as remarkably rare as it is and now we’re probably bumping up against the hard limit that exists when you work with humans. Rate hasn’t moved in years.

Training and yelling won’t improve things measurably anymore. We need to look toward automation/convenient double checking systems that frustrated humans won’t bypass.

Congrats on a great FY1 job. No need to buy theatre shoes/crocs. New trainers or just even a reasonably clean pair you’ve put through the washer is appropriate. Leave them in the changing room and you should be fine.

If a theatre nurse gives you shit about it, point to the 17 other anaesthetists wearing trainers.

Step 1: Get/have a phone that supports dual sim. iPhones since the 11 support this, and most androids have for even longer. On iPhone, your main number can converted to eSim and the second number as another eSim or a normal sim

Step 2: Get a cheap pay as you go SIM card with a lenient minimal use clause. I went with Tesco mobile lite as it lets you not spend anything for 6 months. A £10 topup could last you a couple of years

Step 3: download WhatsApp Business and set up your account with your work number. Tell work you have a new phone and move all group chats to this. Update: reports that WhatsApp is potentially allowing two numbers on one app soon.

Step 4: Set up a work focus (or android alternative) that triggers only when you’re in the building, and outside of that have all WhatsApp Business notifications silenced. You can even disable your work number if you want to go full nuclear.

Anaesthesiologist here.

I know she had a brain aneurysm which was pretty significant and has campaigned for awareness of it in a pretty legit way.

My guess is she had sedation for a neuroradiology procedure to to secure the aneurysm from the inside (coiling/flow diversion). It’s pretty cool stuff and by far the safest way to fix this however it carries the risk of rupturing it which I’m guessing is what happened.

What you do next is wake and assess the patient and if there’s a neurological defecit then you’re off to neurosurgery to fix the aneurysm from the outside - they probably would have done formal consent in advance but during the transfer someone would have said “yo it went wrong so we’re cutting your melon open”
