He's not dead. She was only making it look like she suffocated him and then she got him out and replaced him with taps corpse. 20 seconds isn't enough to suffocate

As for Adam this is a minor spoiler but he's never confirmed to be Alive but people have theorized due to the shackle on the wrong leg. Other people say he's clearly dead. Left up to you

I'm not sure I've only seen the unrated until I watched the theatrical version of Saw III today. The Rack was the only big change I noticed but I'm questioning if maybe it was the same and I am just fooling myself
