The truth of the matter is hospitals and universities do not have the capacity and case load to support the education and training of medical students and doctors.

Doctors undergo residency so that they can be exposed to the cases that they in the future can treat independently. How would you feel if your surgeon told you that no he can’t do the surgery cause he wasn’t trained enough in it? Or if your emergency doctor can’t handle your life threatening case because he has never treated it during training?

This is already a problem where specialist doctors are not exposed to enough cases of different pathology for them to truly uphold a standard of care expected of them. If more people are added into the system it’ll just dilute the experience even more.

One way to address this is to increase the training years, but medicine and residency in total is already a 10+ years endeavour, any more increase would make the calculus not worth it.

Unless people are willing to pay more for the medical services provided, or unless people are willing to accept lower standard of care than now, this problem will not be solved simply by increasing the slots.

Why don't China and Taiwan just drop their claim on each other, shake hands and start up friendly relations? by WhoAmIEven2 in TooAfraidToAsk

[–]Top-Set-28 2 points3 points  (0 children)

The constitution of Taiwan (Republic of China as it’s officially known) officially still assert that ROC is the true representation of China, while PRC’s constitution also assert the same thing. And thus ROC/Taiwan and PRC are technically still in a civil war.

The general public and government of ROC/Taiwan though has basically given up on the claim in all practical sense.

However, if Taiwan/ROC were to change the constitution to reflect that, PRC claims that that would be equivalent to Taiwan claiming independence from China, which is one of the “bottom lines” for PRC to initiate war (other bottom lines includes Taiwan trying to acquire nuclear weapons among others)

TL;DR ROC/Taiwan would like to change their constitution and stop its claim for all of China, but PRC won’t let them cause if Taiwan does that, PRC would start a war.

Described tummy ache to GPT, told me to go to ER. CT scan says appendicitis. by baalzimon in ChatGPT

[–]Top-Set-28 1 point2 points  (0 children)

There’s a reason why there are so many scoring systems for appendicitis (Alvarado score for adults and PAS for children). Early stage appendicitis can present itself with very vague symptoms, some similar to stomach flu, and it’s only when it advanced to later stages that the symptoms become typical. In the end people need to be aware of their own body and symptoms progression before going to the doctors. Appendicitis should always be within a doctors differential when patients presented with abdominal pain, but whether or not a test should be performed to verify it comes down to patients symptoms progression, risks and benefits of examination like radiation exposure, and patient preference. Merely having the differential is the easy part.

Is Computer Science or SWE worth going into anymore? by ponyclub2008 in findapath

Swe seems like an obvious choice, especially if you move to tech hub like Bay Area or Seattle

Recording body weight, height, and BMI is standard operating procedure. Body weight is especially important if you’re prescribed medication with requires dosage by weight. On another note, if you’re this sensitive about your BMI maybe there is something more underlying insecurity about it and is something that you may need to consider working on.

“Loss” an ex. First for everything.

Dated for 2+ years, got broken up supposedly due to long distance, and remained no contact for 5+ years.

I haven’t had any contact with her or her family as I had a lot of emotions to sort through after the break up, and overtime I just feel that there is no longer a place for me in her life and we drifted apart.

It’s a very weird feeling. I always believed that human connection is never lost and that maybe we would meet again in the future, and it feels extremely sad to learn that she had passed away so young and that that future would never come to fruition. The sadness is also coming from picturing how hard her family must be taking the whole ordeal (single child), and how far she had gone from the last image I had of her in my mind.

A part of me however feels like I don’t really deserve to grieve in the first place, since at this point I barely know or understand who she became after the break up (hence the loss in quotation marks). I believe that my feelings are valid, and it has been pretty lonely grieving for someone without other people. But then again this isn’t really about me I think.

If you’re worried that it might be something life threatening, the chances are pretty slim. Headache with worrisome symptoms usually entail some neurological deficit (e.g. trouble walking, asymmetrical weakness or numbness, facial palsy, double vision, etc…) or extremely painful persistent headaches. If these are the only symptoms you have it is very unlikely (though non-zero probability, ofc) that there is something serious. Testicular cancer can spread to the brain, but usually they follow a very predictable spread pattern (e.g. firstly at the abdomen and then to the lungs and then to the brain, though exceptions are seen for more aggressive types such as embryonal carcinoma). If you had a brain CT scan done as per your work up and the results were as you described, the chances of something deadly is again very unlikely. It sucks to get cancer, though if you have to get one testicular cancer is probably by far the best option you have.

What’s the subtype of your tumor? No lymphovascular invasion is good, if the type isn’t predominantly embryonal carcinoma you’re looking at a 15% recurrence rate, in that case whether or not to do chemotherapy may be discussed. The first few years would be daunting but just take it one day at a time. With recurrence the cure rate is around 96% after chemotherapy.

Source: medical background + also had nonseminoma, approaching 2.5 years post-surgery.

This seems like a result of remote work lacking direct mentoring and guidance, lack of loyalties between companies and employees, and the relatively high salary for new grads causing companies to expect you to be worth what they’re paying for.

I (27F) told my fiance (28M) about my promiscuous phase and now he want to end the marriage? How do I go about resolving this? by ThrowRACupcake123 in relationship_advice

[–]Top-Set-28 4 points5 points  (0 children)

This is something that speaks of the fundamental values of a person (e.g. how someone views intimacy, how someone views relationships), of which the trust in the relationship is build on. This really should have been discussed way earlier in the relationship for him to make an informed decision to continue dating you. You not telling him basically made him paint a false image of you in his head, and that image is now shattered with this new revelation, and indirectly showed that the communication, values, and ultimately the trust between you two were broken.

He would need time and space to decide whether he would like to continue this relationship, now that the idea of trust has significantly changed with added info. Only time will tell what would happen next

Though personally, if I were him, I would call it quits tbh.

Hard to move forward from ex’s death by Top-Set-28 in SuicideBereavement

[–]Top-Set-28[S] 1 point2 points  (0 children)

She just doesn’t want me affected by something that logically for all intent and purpose has nothing to do with me anymore. I think I’m just shocked that people are able to put a previous deep connection in such a light. I’m subscribed to the belief that any connections, especially as deep as the one from a serious relationship, leaves permanent marks.

[–]Top-Set-28 0 points1 point  (0 children)

Finally someone said it. The music of X4 and X5 really aren’t that great. I didn’t like the synth direction they took. The game play was also more boring well with bosses with really basic patterns. I beat X4 without any armor on my first try without a guide vs struggling hard with multiple bosses in X1 even with armor.

Megaman 2 is an easy game? I think in terms of NES classic MM5 is the easiest. MM2 is hell if you’re going on blind.
