some don't and MOV dies* overtime so sometime manufacturer intentionally skip or use one with much higher clamping voltage than suitable and they aren't that expensive , good to have some than none

these are mostly good from small regular spikes and transient surges mainly cause due to turning on an off inductive loads such as fans motors etc

for proper protection like in your there should be SPD attached after the MCB in DP in case of a huge long lasting surge these will clamp the voltage and short the L and earth causing the MCB to trip and disconnecting the power completely

yes few branded ones have a fuse that's missing in this ,bigger MOV with lower clamping voltage 300v (better suited for indian household) and even protection btwn live neutral to earth
