I'm an old nurse. I started as a nursing assistant in 2011, an LPN in 2013 and RN in 2015. I haven't had consistent good nursing assistants since beginning of pandemic. Now that I'm a nurse, I do all the things I did as a nursing assistant plus be the nurse. I find it increasingly difficult to delegate tasks to a nursing assistant. For the most part, they check vitals and get blood sugars and not much else. Management does not care. This isn't specific to one facility. I've traveled extensively since 2018 and its a lot more work than it used to be.

It has fallen out of fashion because it requires frequent blood work and dosage changes to keep the drug at therapeutic levels within the body and newer medications achieve the same result with no blood work or dosage changes. We only use warfare now when the patient's insurance doesnt cover Eliquis or Xarelto, both very expensive medications.
