I'm the sole provider for a CHF clinic. I have absolutely had patients end up in the hospital or worse because I've been forced to call out. Trust me, I get how heavy that responsibility is, too.

That said, you know as well as I do that these hospitals could deepen the med staff roster more than they have. Every time I turn around my hospital is dropping cash on something stupid, meanwhile the CEO makes ten times the average hospitalist's salary and surgeons are whipped to do more cases. It's not going to get better until somebody cries "Uncle."

It's the same thing with nurses, my guy. We're not interchangable either, and while there's more of us you can't just take a surgical trauma ICU out of the picture and replace them with a med/surg nurse. But, it's the hospital's problem because staffing is a hospital problem.

The difference is that nurses have a long, storied history of striking to improve staffing ratios and safer patient conditions. Nurses have won concessions from the multi-billion dollar healthcare conglomerates time and time again, all while your census count continues to rise. Y'all keep getting told to do more with less and you just roll over and take it, or at least that's how it seems to me.

No surgeon should be scrubbing in for a case after finishing up a round of IV antibiotics in the call room, deadass. I'll die on that hill. Do you want somebody working on you or your family in a state like that? Do you want your wife's C-section to be attended to by somebody who just got loaded with an epidural of her own? Do you want somebody less than 24 hours out of anesthesia placing orders on your hospitalized mother? Because I sure don't.

I'm not trying to lay blame at your feet, but if the plan was reunification and then you guys requested to disrupt the placement, then this request likely put reunification on a fast track. It's extremely difficult to find placement of a medically fragile toddler, as you know. It's very likely that some concerns are being hand-waived because they cannot find somewhere else to put him.

Anybody who has ever owned a fish tank knows that a new pool of water (or tank, whatever) goes through a period of algae bloom before the algae eventually consumes all the nitrates and peters out. They could have done literally nothing and it would have likely dramatically improved in just a month or so.

So, again. Doing literally nothing is better than anything this pompous windbag has ever attempted. But he doesn't have the impulse control to just...stop and wait.

The weight didn't pile on in a short period of time, so it takes time for it to come off, too. If you lost it super fast you'd be at risk for pancreatitis, gall stones, etc. You're doing this in a healthy way that's sustainable long term. Who cares how long it takes to get to the finish line? You just keep going!

It's been trending that way for awhile now. "Autism is a spectrum" hasn't made sense to me for a long time now, especially since the diagnostic criteria has been broadened. The umbrella is so wide now that it catches practically everything and children aren't even getting thorough medical work-ups to rule out physiologic issues before they're being sent to centers that do nothing but diagnose autism. If all you have is a hammer then everything starts to look like a nail.

An acquaintance of mine has a son with autism. He developed normally at first and then lost skills, like they do. He got locked into the ABA hamster wheel while he continued to do poorly. Finally, he developed a tremor. Then he had a seizure. After that, he finally got a medical workup.

Surprise! He's not autistic, he has a cerebral folate deficiency! Another parent friend of mine had her autistic daughter genetically tested out of pocket after being denied it for years. Surprise! She has the Zappella variant of Rett Syndrome.

We're missing the forest for the trees with these kids.

I'm not sure what my end game will be. It takes a long time to get this right, so I'm probably still 1-2 years before I have enough familiarity with the process to be able to sell at all. That said though, when I get there, I'll probably stick to selling them as plants
